What the Evidence Says About Physiotherapy

The research here is genuinely encouraging. Studies of knee osteoarthritis have found that a large majority of patients, in many trials around 70 percent, who commit to a structured strengthening and rehabilitation programme are able to avoid or significantly delay surgery. Strong evidence also shows that for certain meniscus tears, physiotherapy produces results comparable to surgery. In other words, for a great many knees, rehabilitation is not a second-best option; it is the recommended first choice.

When Surgery Is Genuinely Necessary

Physiotherapy is powerful, but it is not always enough, and being honest about that matters. Severe, end-stage osteoarthritis (grades 3 and 4) with bone-on-bone changes, complete ligament ruptures in active patients, and large mechanical meniscus tears that lock the knee often do need surgery for the best outcome.
